These are just some of the findings of the biggest
study of its kind into migrant dentists in Australia, conducted by the University of Adelaide's Australian Research Centre for Population Oral Health in the School of Dentistry.
The results of a survey of more than 1000 migrant dentists
have been published in the International Dental Journal.

Madhan Balasubramanian, Research Associate in the School
of Dentistry. Speaking in the lead up to World Oral Health Day (Friday 20 March),
Mr Balasubramanian says Australia continues to remain a popular destination for migrating dentists.

The majority of dentists in this group are younger, and
a larger proportion work in the more disadvantaged areas of the country, including rural and remote Australia.
He says this also raises ethical issues about whether Australia should
accept dentists from developing countries. The recently introduced WHO Global Code for International Recruitment for Health Personnel is a significant global diplomacy tool, providing a framework to address
the challenges related to dentist migration. This seems more relevant
in student and academic migration.


Prior to the actual procedure, dentists in Cape Coral would often check when the patient is suitable to undergo the treatment.
Once the dentist is indulged then the dentist would often schedule the procedure.
Before the teeth whitening procedure actually happens the dentist would
place a rubber shield or gel on the patient's
gums to protect the soft tissues. Once this is done,
whitening product is applied on the teeth by using a
specially produced tray that fits into the patient's mouth like a
gum-shield. Most of the whitening products used contains hydrogen or carbamide peroxide that, as it breaks down the oxygen, penetrates into the enamel of the teeth.



This then causes the teeth to lighten. The entire teeth whitening procedure generally takes
about three to four weeks because the patients would require to have several visits to the dentist prior to the procedure itself.
On the first few visits the dentist needs to create a
mouth guard which is made from an impression developed
from the patient's mouth. Oftentimes, patients who undergo

A whitening product would be given to the patient after the treatment to maintain the
effect. The regularity of the application of the product depends on the dentist's advice.
The effect of a teeth whitening procedure generally lasts for three to four years.

However, the length would depend entirely on the patient's oral hygiene and practices after the procedure.

assess the status of your teeth, including taking a
dental history, which may have a bearing on the treatment they're prepared to give you.
They can also assess and redress underlying problems causing discolouration. They're more likely to offer custom-fitted bleaching
trays, which means better control of the bleach and less gum irritation. 1000, give or take

Unlike in a salon, dentists are also allowed to do what's called 'power bleaching', which uses strong hydrogen peroxide bleach (up to 38%
hydrogen peroxide). It's placed in trays and fitted to your teeth, and the bleaching effect is often enhanced with the use of a light or heat source,
such as laser, LED or halogen lamps. In general, the stronger the product
and the longer the contact, the whiter the result. However, teeth are
also likely to be more sensitive afterwards.
Using light or heat to enhance the effect of the bleach appears to have a greater whitening effect than bleach alone.
However, much of this extra effect appears to be due to dehydration of the tooth, and the colour will darken slightly as the tooth rehydrates.



The effects of power bleaching are immediate,
but more than one treatment may be needed to get the desired results.
It's not uncommon for an initial treatment at
the dental surgery to be followed up with further treatment or maintenance at home.
Your dentist can also provide home bleaching kits,
either as a stand alone bleaching process or to supplement
in-office power bleaching. These are likely to be more expensive, simply because the dentist will have made custom-fitted trays, though you can be reassured
the strength of the product will be appropriate for you. Be prepared for increased
sensitivity Using the product less often will help, though the whole process will take longer.

A sensitive toothpaste and/or Tooth Mousse can reduce sensitivity.



Don't over-bleach - don't use more product (it won't
help), leave it on too long or do it too often. And
you only really need to bleach the front surface of your
teeth. Choose products from reputable Australian or US suppliers Don't be tempted to save a few dollars buying unreliable products subject to
poor hygiene and quality control. There's also plenty of scam companies out
there, so check the credentials of the company first.

Work on one set of teeth (top or bottom) at a
time You'll notice the whitening effect more
if you have a point of comparison and wearing one tray is more comfortable than wearing two.



Most whitening products (except toothpastes) will contain peroxides,
such as urea peroxide or carbamide peroxide.
This breaks down to hydrogen peroxide (10% carbamide peroxide releases 3.5% hydrogen peroxide in the presence of water) which chemically bleaches both surface stains and the tooth itself.

At a high enough concentration, peroxides can whiten teeth so that
they're lighter than your natural colour. DIY home tooth-bleaching kits can be easily purchased via the internet, with mixed reviews.
They may also come as a tooth-whitening pen or adhesive strips.
When DIY teeth-whitening kits initially hit the market, CHOICE found products with hydrogen peroxide levels as high as 16% (carbamide
peroxide 44%). This was despite recommendations by the Australian Dental Association that
home teeth-whitening products not exceed 6% hydrogen peroxide.
Regulations now limit the strength of DIY kits
to 6% hydrogen (18% carbamide peroxide).


However, dentists may still sell stronger kits for in-home use, based on their professional judgement - the 6% is based on risks of toxicity,
rather than the effects on your teeth. Try to avoid bleach contact with gums as much as possible,
and avoid swallowing the bleach. There is some concern, though no conclusive evidence, that peroxides may cause oral cancer, though it's more likely in people already at risk - smokers and those who
abuse alcohol. Excessive bleaching attempts can damage enamel, as can overly acidic
products. You may end up with more porous enamel or a bluish tinge.



Some people can also experience temporary tooth or gum sensitivity when using home bleaching products.
The fit of the bleaching trays is an important aspect of this
sort of teeth whitening. If you're leaving them on for a long time (30 minutes or so),
they need to be comfortable, and not cause gagging or excessive salivation. And the closer
they fit, the less bleaching product you need to use.
Excessive bleach can ooze out, and while not toxic in small
amounts they can taste unpleasant and may damage soft tissues.
Pre-filled trays promise the convenience of not having to muck around with
tubes of bleaching gel and so on. But because they're one-size-fits-most, there needs to be a lot of gel in them
to make sure all teeth have contact with the active ingredient.
Excess gel oozes out and irritates the gums.



Most tooth-whitening products will use some sort of abrasive to remove stains, but
some also use other ingredients. Abrasive agents (such as alumina, silica/hydrated silica, dicalcium phosphate) remove surface stains
by physically rubbing them off the surface of the teeth.
The levels of abrasives in toothpastes are generally low, so you shouldn't need to worry about them wearing down the surfaces of your teeth.
Detergents (such as sodium lauryl sulphate) act as foaming agents and
help to clean the tooth surface. Enzymes (such as papain, derived from papaya) slow down the build-up of
'pellicle' — the protein-containing layer of saliva that forms on your teeth within a
minute of brushing. Surface stains stick to the pellicle layer.
Polyvinylpyrrolidone (PVP) acts to prevent stains from accumulating on the tooth surface.

Interestingly, it was also the main ingredient in the first really successful hairsprays in the 1950s!



Triclosan is an antibacterial agent that can help reduce plaque levels and fight tooth decay.

It's also an anti-inflammatory agent, so can reduce the progress of gum diseases.

that they're less able to stick to your teeth. Developed initially by a team of
dentists and scientists at the University of Melbourne,
Tooth Mousse has taken the dental world by storm.
Tooth Mousse contains calcium and phosphate derived from milk protein (casein), and works to remineralise the enamel surface of the tooth,
preventing and reversing surface damage. It's recommended
for tooth erosion, early decay, fluorosis, lesions due to orthodontic treatment and tooth sensitivity - including post-bleaching sensitivity.
By strengthening the enamel, it also protects against erosion and caries, and can be used pre-whitening
to prevent sensitivity.
